Support Planning at Unity Neighborhoods Hub is for individuals and families who want a clear, realistic roadmap instead of constantly reacting to crises. We know how it feels to juggle appointments, behaviors, and daily needs with no real plan, because many of us have lived it in our own homes.

We start by listening. We learn what everyday life looks like for you right now - what is going well, what is exhausting, and what feels scary or out of control. From there, we work together to create an individualized support plan that fits your strengths, needs, and routines rather than forcing you into a one-size-fits-all approach.

A key part of this service is Behavioral Support. We help you understand what might be behind challenging behaviors, then build positive coping strategies that are gentle, consistent, and doable. That might look like creating calm-down plans, visual schedules, clear reward systems, or new ways to respond during meltdowns or shutdowns. We always focus on safety, dignity, and respect for the person at the center of it all.

Your plan can include goals around daily living, health, communication, school or work, and community activities. We break big goals into small steps, assign who will do what, and decide how we will track progress together. We also help you prepare for meetings with schools, doctors, or service providers so your voice is heard.

Support Planning is not a one-time document that sits in a folder. We revisit and adjust it as life changes, offering ongoing guidance so you are not left to figure it out alone.
