Community Care at Unity Neighborhoods Hub is about making sure no one is shut out of everyday life just because systems or spaces were not built with them in mind. We focus on helping individuals with disabilities - and their families - feel more confident, connected, and included, both at home and out in the community.

Through Social Skills Development, we practice real-life interactions in a relaxed, supportive way. That can include greetings, turn-taking, asking for help, handling disagreements, or joining group activities. We use role-play, visuals, and step-by-step coaching so skills make sense and can be used in familiar settings like school, work, church, or neighborhood events.

Our Community Inclusion Support is all about opening doors. We help identify activities the person might enjoy - such as recreation centers, volunteer spots, social groups, or faith communities - and then plan how to get there, what to expect, and how to stay involved. When needed, we can attend early visits, talk with staff about accommodations, and suggest simple changes that make participation easier and more comfortable.

Family Support Services are woven through everything we do. We know caregivers carry a lot, often with very little backup. We offer practical resources, emotional space to talk things through, and coaching on how to support communication, behavior, and independence at home. Our aim is to help families feel more like a team and less like they are barely keeping up.

Services can happen at home, in community settings, or virtually, depending on what works best. Every plan is shaped around the person's interests, abilities, and cultural background.
